Suite224 Internet's helpdesk will review the information you have submitted for service type and speed. This application was built to get the information we need from you to start the process for your Internet service installation. Suite224 reserves the right to select the best service type and / or method of service delivery. If broadband is available at your address from Suite224 we will determine the broadband medium (Cable or DSL) and present you with the billing information accordingly. Broadband pricing varies from one telephone company to the next.

This agreement is entered into this day of 02/04/2012 by and between The Conneaut Telephone Company and applicant (yourself). In order to become a member and use the services of Suite 224 Internet, a division of The Conneaut Telephone Company, I hereby agree to Suite224 Internet's Acceptable Use Policy , Terms and Conditions, and the following:
Suite 224 Internet may only be used for lawful purposes. Transmission of any material in violation of federal, state, or local laws, regulations or ordinances or in criminal transactions is strictly prohibited. The service may be immediately disconnected upon violation of this paragraph. I agree to indemnify and hold harmless Suite 224 Internet from any claims resulting from my use of the service which damages me or another party, including data loss from service interruptions, its own negligence, and service delays. Use of information obtained via Suite 224 Internet is at the risk of the user. Suite 224, its officers, directors, agents and/or representatives exercise no control over data transferred or made available. Suite 224, its officers, directors, agents and/or representatives also disclaim all warranties and will attempt to keep your material confidential; however, Suite 224, its officers, directors, agents and/or representatives cannot guarantee the privacy of your mail and/or data.This agreement and the service provided thereunder will continue in effect until either the undersigned or Suite 224 Internet gives written notice of termination at least 24 hours prior to termination. This notice may be posted via e-mail and/or U S mail to the most recent address available. Out of consideration for other users, any connection not having active participation for more than 15 minutes is subject to disconnection. The user may reconnect at any time.Suite 224, through the Internet connection, creates access to materials which may not be suited to minors. Consent of a parent or guardian is required for users under 18 years of age. Suite 224, through the Internet connection, creates access to materials which may be deemed personally offensive or inappropriate. You agree to hold harmless Suite 224, as it is a provider of connections, and not of content. If any part of this agreement is deemed to be illegal and/or against public policy, the remaining portions of this agreement will remain in full force and effect.

Disclaimer of Service

Suite 224 Internet is in the business of supplying the customer with an Internet connection. This consists of the necessary network address numbers and the telephone number the customer calls to access Suite 224 Internet. Suite 224 Internet does not support 3rd party software, your hardware, or peripheral devices attached to the customer's computer. Suite 224 Internet technical support staff will help the customer configure his computer to connect to Suite 224 Internet. This consists of supplying the Username, password, telephone number, and necessary network address numbers and instructing the customer where to insert this information in the connection software.

14 Day Connection Window

Suite 224 Internet will make every effort to help you connect to the internet upon account activation. It is your responsibility to contact Suite224 technical staff if there is a problem. If your system is unable to connect upon account activation, and you do not notify Suite224 technical staff within 14 days, you will be billed the connection fee and the first month's access fee. You will then be responsible for the bill. You will be billed for service if you do not contact the billing office at 593-7140 within this 14 day period. The 14 day period starts when your account is activated in the system.

On Site Support Policy

Suite 224 Internet will make every effort to configure your system over the telephone, but on rare occasions, it may become necessary for a technical support representative to come to your house. This decision will be at the discretion of Suite 224 Internet's technical support team. On site service is billed at the rate of $50 per visit. Suite 224 Internet will ensure that the Internet connection is operational before leaving, and is not liable for any other computer hardware or software conflict.

Non-payment Disconnection Policy

Bills will be mailed to our customers on the 1st of the month. Payments are due on the 15th of the month. If the bill is unpaid by 12:01 am on the 21st of the month, the account will be terminated. To re-establish a terminated account, there will be a $20.00 reactivation fee.
Typing your name below constitutes an electronic signature which shall be sufficent to verify that you have read, understood, and agree to the above policies and agreements, as well as to the Suite224 Internet's Acceptable Use Policy, and Terms and Conditions. This electronic record, and your electronic signature shall be deemed for all purposes a.) to have been "signed", and b.) to constitute an "original" when printed.
The Suite224 Internet Acceptable Use Policy and the Suite 224 Terms and Conditions are integral parts of this agreement. Please read them carefully before signing below. We reserve the right, at our discretion, to update or revise the Acceptable Use Policy and the Terms and Conditions. Please check the Suite224 website periodically for changes. Your continued use of Suite224 Internet Services following the posting of any changes to either the Acceptable Use Policy or the Terms and Conditions will constitute your acceptance of these changes.
